Rebuild Login/signup popup and form

I’ve seemed to completely bugger up the stock login and signup forms. How do I get new one? Is there a way to revert?

I can’t confirm this at the moment, but there should be a Signup Login form template (at the very bottom of the left hand element list below reusable elements) that should generate a fresh form every time you add it. I could be very wrong, but I think it’s different from the built-in reusable element.
